Asheville Rally ICE list, getting that time again

I am again wanting to do an attendance list of emergency numbers for folks coming to Asheville this year. Thankfully, we did not have to use the info for a crash situation, but it was used a couple of times to retrieve a bike that had a mechanical problem, and some coordinating of meet-ups.

I would like to use this format so I can just do a cut & paste: It also helps if you keep it a simple font and no color.
